This Website Does Not Create a Provider-Patient Relationship

Browsing this website, submitting the contact or appointment request form, or otherwise communicating through this site does not, by itself, create a provider-patient relationship with The Florida NP Clinic, LLC or The Florida NP Clinic, LLC. A provider-patient relationship begins only once you have completed intake and been accepted as a patient through our scheduling and practice management platform, Carepatron.

Not for Emergencies

This website and its contact form are not monitored continuously and must never be used for medical or mental health emergencies. If you are experiencing an emergency, call 911 or go to your nearest emergency room. If you are in crisis, call or text 988.

Telehealth Eligibility

Care described on this website is delivered exclusively by telehealth to patients who are physically located in Florida at the time of their visit, consistent with Kirsis Perez's Florida licensure. We cannot provide care to individuals located outside Florida at the time of the visit.

Content on this website, including descriptions of services and conditions treated, is provided for general informational purposes only and is not medical advice. It is not a substitute for an individualized evaluation by a qualified healthcare provider. Appointment scheduling and certain intake and records functions are provided through Carepatron, a third-party platform. Your use of Carepatron is subject to Carepatron's own terms of service and privacy policy, in addition to these Terms of Use.
