Before your visit

• Be physically located in Florida at your appointment time. This is a licensure requirement.
• Have your medication list ready, with doses, including supplements and anything over the counter.
• Gather any recent lab results or records from other providers.
• Know which pharmacy you use.
• Find somewhere private with a decent connection.
• Write your questions down. Visits go better when you are not trying to remember them on the spot.

How joining works

1. Book the free membership call, or send the Become a member form and we will contact you. You get a short form first to confirm you will be in Florida for visits and are not enrolled in Medicare.
2. On the call, go through how the membership works and what it costs for your household.
3. If you decide to join, read and sign the membership agreement. Your first month and enrollment fee are charged only after it is signed.
4. Complete your health intake forms.
5. Book your first visit with Kirsis Perez, ARNP, FNP. Visits open once the agreement, payment and intake are done.

Service area

All of Florida. Licensure is statewide, so no part of the state is excluded. There is no physical office to visit.
