Membership is $79 a month for ages 18 to 44, $99 for ages 45 and older, and $35 for children 17 and under, and a family plan covering two adults and two children is $219 a month. Each adult pays a one-time $99 enrollment fee, children are not charged one, and there is no charge at any visit. Laboratory testing is billed separately at the practice's own cost. A membership is not health insurance.

Membership runs month to month. Either side can end it with thirty days' written notice, and fees you have prepaid are refunded if the practice ever stops offering care. The written agreement states all of this before you sign.
Eligibility
Who can join
Anyone in Florida at the time of each visit
You need to be physically located in Florida during every appointment. That is a licensure rule. Where you live does not matter; where you are sitting during the visit does.
All ages
Adults join on their own. Children join together with an enrolled parent or guardian, who takes part in their visits.
Not currently available with Medicare
Memberships are not available to anyone enrolled in Medicare, including Medicare Advantage plans, because of how federal Medicare rules apply to practices that do not bill Medicare. Your Medicare coverage already includes primary care visits. If a member enrolls in Medicare later, the membership ends with thirty days' notice, prepaid fees are refunded, and records go with them.

Member prices are what The Florida NP is billed and passes through without markup, billed separately from your monthly membership. The walk-in column is the published national range for a laboratory visit without insurance or a practice account. Prices change, and nothing is ever ordered without telling you what it costs first.
Paying from a health savings account
Since January 1, 2026, federal law allows a qualifying direct primary care arrangement alongside a health savings account, with the monthly fee payable from the HSA up to $150 a month for an individual and $300 for a family. Every individual price and the family plan are set under those limits, and a family plan stays under the family limit with up to 4 children. Federal guidance does not specifically address one-time enrollment or rejoin fees, so confirm how to pay those with your tax advisor or plan administrator. How much does a membership cost?
Membership is $79 a month for ages 18 to 44, $99 for ages 45 and older, and $35 for children 17 and under, and a family plan covering two adults and two children is $219 a month. Each adult pays a one-time $99 enrollment fee and children are not charged one. There is no charge at any visit, and laboratory testing is billed separately at the practice's cost.
How is the membership billed?
Monthly, to a card or bank account kept on file. The first charge is your first month plus the enrollment fee, and nothing is charged until you have read and signed the membership agreement.
How does the family plan work?
The family plan is $219 a month for two adults and two children 17 and under in the same household. Each additional child adds $35 a month, and any other adult, such as a grown child, parent or roommate, adds their own adult tier. If separate memberships would cost your family less, you are charged the lower amount instead. One-time enrollment fees are separate.
What is the rejoin fee?
If you cancel and later decide to come back, rejoining is $99 per adult. A membership works because people stay enrolled between illnesses, not only during them, and the fee keeps the monthly price low for everyone who does.
Can I join if I have Medicare?
Not at this time. Memberships are not available to anyone enrolled in Medicare, including Medicare Advantage plans, because of how federal Medicare rules apply to practices that do not bill Medicare. Is the membership call really free?
Yes. It is a 15-minute call with our membership team about how the membership works, what it costs for your household, and whether it fits. It is not a medical visit, so health questions are saved for your first visit with the provider. There is no charge and no obligation.
Do you bill my insurance for anything?
No. Nothing covered by the membership is billed to a health plan. You can still use your insurance for care outside the membership, such as specialists, imaging, hospital care, and prescriptions at your pharmacy.
Required notice
A direct primary care membership with The Florida NP LLC is a direct health care agreement under section 624.27, Florida Statutes. This agreement is not health insurance and the health care provider will not file any claims against the patient's health insurance policy or plan for reimbursement of any health care services covered by the agreement. It does not satisfy any requirement to maintain health coverage.
