Our Commitment to Your Privacy

The Florida NP Clinic, LLC, operating the practice of The Florida NP Clinic, LLC ("we," "us," "our"), is required by law to maintain the privacy of your protected health information ("PHI"), provide you with this notice of our legal duties and privacy practices with respect to your PHI, and follow the terms of the notice currently in effect.

How We May Use and Disclose Your Health Information

Treatment

We may use and disclose your PHI to provide, coordinate, or manage your healthcare, including sharing information with other healthcare providers involved in your care, such as specialists we refer you to or the laboratory that processes your labs.

Payment

We may use and disclose your PHI to obtain payment for services provided to you, such as verifying insurance coverage or billing.

Healthcare Operations

We may use and disclose your PHI for operations necessary to run our practice, such as quality assessment, staff training, and practice management activities, including through our scheduling and records platform, Carepatron.

Other Permitted or Required Uses and Disclosures

We may also use or disclose your PHI, generally without your written authorization, when required or permitted by law, including:

As required by law, court order, or subpoena
For public health activities, such as reporting communicable diseases
To health oversight agencies for oversight activities authorized by law
To avert a serious threat to health or safety
For law enforcement purposes, as required or permitted by law
To coroners, medical examiners, or funeral directors as necessary
For workers' compensation purposes, as authorized by law

Other Uses Require Your Written Authorization

Uses and disclosures not described above, including most uses for marketing purposes and any sale of PHI, will be made only with your written authorization. You may revoke that authorization in writing at any time, except to the extent we have already relied on it.

Your Rights Regarding Your Health Information

Right to Access: You have the right to inspect and obtain a copy of your PHI in our records, with limited exceptions.
Right to Amend: You have the right to request that we amend your PHI if you believe it is incorrect or incomplete.
Right to an Accounting of Disclosures: You have the right to request a list of certain disclosures we have made of your PHI.
Right to Request Restrictions: You have the right to request restrictions on certain uses and disclosures of your PHI. We are not required to agree to all requests.
Right to Request Confidential Communications: You have the right to request that we communicate with you about health matters in a certain way or at a certain location.
Right to a Paper Copy: You have the right to a paper copy of this notice upon request, even if you agreed to receive it electronically.
Right to File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

Our Duties

We are required by law to maintain the privacy of your PHI, provide you with this notice, and abide by its terms. We reserve the right to change this notice and to make the revised notice effective for PHI we already have as well as PHI we receive in the future. An updated notice will be made available on this website and, upon request, in our office.

You may file a complaint with our Privacy Officer at the contact information below, or with the Secretary of the U.S. Department of Health and Human Services at www.hhs.gov/hipaa/filing-a-complaint.
